This appointment will focus on an area causing pain or infection, and if appropriate, treatment will be provided the same day. This type of exam focuses on a specific problem, where pain or infection are not involved. Treatment options are discussed and the patient decides or confirms the plan of action.

Please download our Patient Registration and Medical History form. You may fill it in on your computer, save it and send it to us. Alternatively, you may print it out, complete it ahead of your appointment and bring it with you. We will attempt to contact you to provide a "courtesy" reminder for all appointments 2 days before your appointment.
