Your first visit to Baseline Dental Centre may include a comprehensive dental examination. Your dentist will assess your individual dental needs, discuss your dental goals, and how we might work together to fulfill them. Your thorough dental examination may include digital radiographs (x-rays). If you have had x-rays taken recently, please bring them with you on your first visit or contact us to have them sent to us directly.

If you would rather complete the forms at our office, we ask that you kindly arrive 15 minutes before your scheduled appointment.
